Seasonal Influenza Vaccine Safety: A Summary for Clinicians



Contraindications

LAIV should not be administered to: people who have had a severe allergic reaction to the following:
  • Children aged <2 years or adults aged ≥50 years
  • People who have had a severe allergic reaction to a component of the LAIV vaccine. The package insert should be consulted for components.9
  • People who have had a severe allergic reaction to to a prior dose of any influenza vaccine.
  • Children or adolescents receiving aspirin or other salicylates (because of the association of Reye syndrome with wild-type influenza virus infection)
  • Egg-allergic persons
  • Pregnant women
  • People with known or suspected immunodeficiency diseases or immunosuppressed states (including those caused by HIV)
  • Children aged 2 through 4 years who have asthma or who have had a wheezing episode noted in the medical record within the past 12 months, or for whom parents report that a health care provider stated that they had wheezing or asthma within the last 12 months
  • Persons who have taken influenza antiviral medications within the previous 48 hours.

Clinical syndromes

The primary clinical manifestaion of CHD is chest pain from stable coronary atheromatous disease leads to stable angina.
Signs of stable angina are; chest pain, breathlessness after exercise, climbing hills or stairs; symptoms are relieved by rest.

A stable coronary atheromatous plaque may become unstable as a result of plaque erotion or rupture formation of thrombus, causing sudden narrowing of the vessel. This causes an acute coronary syndrom (ACS) which is characterized by chest pain of increasing severity. These patients are at high risk of MI & death & require immediate hospitalization.

What is coronary heart disease

CHD is a condition in which vascular supply to the heart is impeded by a trauma, thrombosis or spasm of coronary arteries.
This impairs the supply of oxygenated blood to cardiac tissue causing myocardial ischaemia which if severe or prolonged may cause the death of cardiac muscles (myocardial infarction).

Myocardial ischemia can also arise if oxygen demand is ↑ such as in severe ventricular hypertrophy (due to hypertension) or iron deficiency anemia (because of decreased capacity of blood to carry oxygen).
